Background

Care360 is a patient management system for Medical Social Workers (MSWs) that supports planning and management of patients’ psychosocial and/or financial care.

Solution

OGP continues to work closely with MSWs from various institutions to design a comprehensive vision and product that:

• Provides MSWs with a more holistic and accurate view of patients
• Enables effective and timely provision of financial and social intervention and assistance
• Reduces routine manual work (e.g. form filling) done by MSWs to allow them to focus providing care to patients
• Enhances the patient experience in the public healthcare system

Impact

Actively used by MSWs across all of Singapore’s Public Healthcare Institutions (PHIs)
